ADEA COSRF Chapter Awards Application

The ADEA Student Chapter Awards celebrate the creativity, leadership and impact of student chapters across the U.S. and Canada. These awards highlight chapters that demonstrate excellence in advancing oral health education, advocacy, innovation and community engagement.

New This Year - ADEA is expanding its recognition of student leadership and advocacy through the Advocacy Day Student Travel Scholarship and ADEA Student Advocate of the Year Award. These awards recognize students who demonstrate exceptional leadership, advocacy and commitment to advancing oral health, higher education, public health or health policy.

Through this application, chapters are invited to share their accomplishments and contributions from the past year while also recognizing outstanding individual student advocates. All applications will be considered for Chapter-Level Designation Awards (Bronze, Silver, or Gold). To be considered for Distinguished Awards (Innovation, Community Impact, Chapter Achievement or Chapter of the Year), applicants must select the award in the applications. Applicants can choose to be considered for one or more Distinguished Award categories:

  • Innovation Award – Honors a chapter that introduces bold, creative ideas that advance dental education and advocacy.
  • Community Impact Award – Recognizes a chapter whose outreach and service initiatives make a measurable difference in communities.
  • Outstanding District Award – Celebrates a district that strengthens collaboration and leadership within its district.
  • Chapter Achievement Award – Highlights a chapter’s significant contributions to oral health education and advocacy.
  • ADEA Chapter of the Year Award – The highest honor, awarded to the chapter that exemplifies overall excellence in leadership, vision and impact.
  • NEW! ADEA Student Advocate of the Year Award – Recognize students who demonstrate outstanding leadership, advocacy and commitment to advancing oral health, higher education and public health policy.

Chapters are encouraged to provide clear descriptions, measurable outcomes and supporting evidence where applicable. Strong applications will articulate not only what was accomplished, but also why it mattered and how it will be sustained. Only one application should be completed per chapter, and only members of the chapter’s administrative board are allowed to submit applications on behalf of their chapter.
